With your consent the following information is collected and updated directly from you, or from an authorized substitute acting on your behalf:
1. the necessary personal health information (eg. name, age, DOB, address, health history, records of your visits and treatmentetc.) for the purpose of providing safe and effective healthcare. We may sometimes collect necessary personal health information about you from other sources, if we have obtained your consent to do so, or if the law permits.
2. the necessary personal financial information (eg. personal cheques, visa, mastercard) for the purpose of administering payment for health services provided.
Uses and Disclosures of Personal and Health Information
Your personal health information will be used and disclosed only for the purposes of:
1. Treating and providing health care to you
2. Receiving or directly invoicing your payment for treatment and health care provided.
3. Conducting risk management and quality improvement activities
4. Complying with legal and regulatory requirments
5. Fulfilling other purposes permitted or required by law.
You Rights
At any time you may access, correct your personal health records, or withdraw your consent for some of the above uses and disclosures(subject to legal exceptions) by contacting Wendy Giles.

Providing you with the best service includes treating your personal health information with the same care and respect as you are treated. Appropriate steps are taken to protect your personal information from theft, loss, unauthorized access, copying, modification, use, disclosure, and disposal.
